What is it about?
Decentralisation of the health system is a common government policy. How this affects health systems is rather unclear. This review brings together all evidence to assess how decentralisation (by devolution) impacts each of the six World Health Organization's (WHO) building blocks, and the health system.

Perspectives
Decentralisation, however well intended, can have unintended consequences. Generally, it is difficult to come to universally applicable conclusions about the pros and cons of decentralisation. However, our review highlights some of the common potential issues policy-makers and other stakeholders should consider in advance.
