What is it about?
A case of dual AV nodal non-reentrant tachycardia (double fire) treated long time for atrial fibrillation. The case is presented as a ECG/EP quiz. The correct diagnosis was suspected after carotid sinus massage and confirmed during EP study. Successful ablation was done.

Why is it important?
This is the first report on using diagnostic carotid sinus massage in this rare tachycardia, dubbed as the 'great deceiver' or the 'great chameleon'. A positive response (as in the case described) could help in the differential diagnosis.
