What is it about?

Describes the steps taken to develop and implement a medication adherence tool that provides a score of adherence to specific medications a patient is taking. This score is based on a proportion of days covered methodology and uses the refill data from pharmacies to generate a percentage adherence score. This score is displayed in the electronic medical record when the patient chart is opened

Why is it important?

Knowing whether a patient is refilling prescribed medications is essential in properly managing prescriptions and disease. This information while in the public domain, is often laborious to obtain. The P-SAM (Parkland Score for Adherence to Medication) score provides this information automatically and seamlessly at the point of care with no effort required by clinicians or other health professionals.

Perspectives

Medication adherence has been one of those issues that has plagued chronic disease management for years. We hope that this practical and effective example of how to bring medication adherence information to the clinician at the point of care catalyzes efforts to better address and manage this challenging issue in healthcare
