Most previously investigated small molecular and protein/peptide biomarkers have relatively weak predictive power for mortality risk after adjustment for demographic and clinical factors in patients with advanced CKD

Why is it important?
This lack of predictive strength could be - at least partly - explained by the elevation of the small circulating molecules and polypeptides along with the decline of GFR. Thus, elevated levels of many biomarkers may reflect not only increased production, but also a prolonged half-life due to impaired renal clearance by glomeruli and the proximal tubules
