What is it about?
FSGS is a broadly encompassing histologic descriptive term that includes genetic FSGS, primary FSGS, secondary FSGS, and FSGS of undetermined cause. Primary FSGS is a podocytopathy that is likely immune mediated and tends to be responsive to immunosuppressive therapies. The proportion of primary FSGS and the overall incidence of primary FSGS is not well defined though a better understanding would help improve our understanding and management. Among a large diverse population in the United States, we identified 3,838 patients with FSGS reported on biopsy. A total of 637 met criteria for primary FSGS. Thus, primary FSGS accounted for only 16% of all biopsy reported FSGS patients. We observed a mean standardized primary FSGS incidence rate of 1.7 (0.9, 2.5) per 100,000 person-years during the 2010-2021. Incidence rates (per 100,000 person-years) were highest among Black (3.2), Asian (2.7), and Pacific Islander (2.8) persons. Primary FSGS appears to be a rare disease.
