What is it about?
This paper investigates the historical connection between the “Form 1 – Application by a Physician for Psychiatric Assessment” used today in Ontario and certificates of insanity employed during the institutionalization period in psychiatry. I argue that the Form 1’s structure carries a remarkable history going back to certificates of insanity printed in Victorian England.
