Who cares? Case management and patient navigation in a public health programme

What is it about?

This research examines backgrounds and orientations of patient navigators and case managers and patient attitudes in a public health program delivering care to underserved communities. Findings identify value of adding patient navigators to improve diversity of care providers and increase community engagement. Ongoing challenges of engaging patents with chronic illness are highlighted.
