What is it about?

This papers explores the critical elements that made a multiprofessional therapist and nurse led rehabilitation ward successful in managing the discharge of older patients effectively. It identifies the value of intraprofessional working.

Why is it important?

We were able to identify the strategies the multidisciplinary team used and developed to make this happen. These included having professionals who were experienced and knowledgeable, were confident in their professional identity and were prepared to work in this way.

Perspectives

Working with leaders/managers and clinicians in the field was a very important part of the process. This was a very positive experience and led to a great sense of possibility in terms of how working differently could in fact make a very bid difference to not only the effective discharge but patient centered delivery and caring and supportive team culture.
