What is it about?
This paper is a systematic review exploring the effect of prehospital hypertonic solutions on isolated traumatic brain injury upon patient outcomes (mortality / neurological outcomes). This is explored through a wide systematic review of the literature.

Why is it important?
Traumatic brain injury account for significant mortality and disability globally. Paramedics are the first responders to these patients but have limited options to manage cerebral oedema and herniation in the prehospital setting, where hypertonic solutions could be the answer. However, my findings suggest that prehospital osmotherapy is a fruitless endeavour having no impact on mortality or neurological outcome. Paramedics should focus on basic resuscitation which is know to affect outcome
Perspectives
I hope this article refocuses the importance of basic principles of resuscitation which are fundamental in all patients, but particular in traumatic brain injury
