Why is it important?
Stroke is increasingly considered a long-term condition that requires appropriate clinical management to optimise patient outcomes and quality of life. There is recognition that people who have had a stroke, and their carers, need to be supported in the months and years following the event. The requirement to identify and address the needs of patients after stroke remains an international priority - but is not always done. We developed a 'tool' that facilitates the identification of needs.
