What is it about?
We describe a rare intravascular large B-Cell Lymphoma case with atypical presentations that eluded a great deal of initial workup. However, maintaining a high clinical suspicion, we diagnosed this disease via muscle biopsy and examination of the perimysial and endomysial vasculatures.

Our case highlights the importance of maintaining a high clinical suspicion while facing rare diseases and thinking outside the box when diagnosing them.
Perspectives
While randomized skin biopsies have long been regarded as the standard for diagnosing intravascular large B-cell lymphoma, the role of muscle biopsies when involved is yet to be discussed. Our case demonstrates that muscle biopsy could be equally valuable, especially when other conventional means, such as non-invasive imaging or bone marrow biopsy, fail to provide the diagnosis.
