What is it about?
Medication-related harm is a major public health problem. The transition back home following hospital discharge is a high risk period for older adults. This article describes the development and validation of the PRIME tool, a tool into which routinely collected patient data can be entered to calculate the risk that the individual will experience medication-related harm requiring healthcare use. This information can be used by clinicians, policy-makers and patients to improve patient safety.
