Use of reagent-strip urinalysis to detect glycosuria remains a routine antenatal care screening practice for gestational diabetes particularly in low- and middle-income settings.

Why is it important?

Variability in glomerular glucose reabsorption during pregnancy makes the diagnostic accuracy of this test questionable.

Perspectives

Glycosuria was found to be insensitive, diagnostically poor and missing majority of cases. Fasting blood glucose was most sensitive while one-hour OGTT was most specific. Our findings reaffirm the rising prevalence of GDM also at primary healthcare levels and highlights the need to monitor pregnant women with the slightest glycosuria even trace results. Fasting blood glucose should be integrated into all GDM screening/diagnostic procedures.
