What is it about?
We describe a new liver resection technique to spare liver parenchyma using the surgical expertise maturated in the field of liver transplantation.

Why is it important?
The most interesting aspect of this innovative intervention is the finding that communicating veins between main hepatic veins can develop spontaneously, after the use of PTFE graft “bridge” solution to preserve temporary the blood-flow in the resected hepatic veins. The observation that the new communicating veins can provide sufficient out-flow, which is followed by the loose of function and thrombosis of the PTFE graft, without affecting the hepatic vascularization and liver function, also represents an original insight on resected liver physiopathology.
Perspectives
The Minor-but-Complex liver resection offers a chance of treatment for patients potentially inoperable or candidate to a major hepatectomy.
