What is it about?
Chronic hypercapnic respiratory failure is often common in advanced COPD as well as hospitalization and death. Long-term non-invasive ventilation (NIV) has been used with uncertain efficacy for this condition. however recent trials suggested that there is a possible survival benefit as well as reduced hospitalization associated with this therapy under specific conditions. The study focuses on updating the existing guidelines by assessing current evidence related to long-term non-invasive ventilation.

Why is it important?
Our findings showed a significant shift from the previous guideline. This guideline update encouraged the use of NIV, in contrast to the previous recommendation which was against the use of NIV for COPD and chronic hypercapnia. Based on our findings on reported survival and hospital readmission rate benefits, we suggested that long-term NIV should be used in highly elected COPD and chronic hypercapnia patients, along with close monitoring and specific use of ventilatory strategies.
