What is it about?

Our research demonstrates that a large number of suspected lymphoma relapses based upon FDG-PET in the course of follow up after therapy for mediastinal lymphomas are due to various other benign or malignant morbid conditions.

Why is it important?

Mediastinal biopsies in cases of suspected relapses of mediastinal lymphoma based upon FDG-PET confirmed the diagnosis in 64.7% of patients. In 35.3% relapse was ruled out. These data are relevant as the therapy for lymphoma relapse is very aggressive. Furthermore the biopsies diagnosed different benign or malignant morbid conditions to be treated .

Perspectives

I believe that the biopsy of suspected mediastinal relapse of lymphoma is relevant both to confirm or exclude the nature of the lesion in order to perform a correct treatment. The biopsy is occasionally easy and frozen section can quickly confirm that a sample is representative of lymphoma, but there are cases where a wide or complete excision is necessary to rule out a relapse. On such occasions surgery can be extremely difficult due to firm adhesions of the lesions with mediastinal structures. A strict cooperation between the haematologist and thoracic surgeon is needed to assess the correct diagnostic plan for each inividual patient.
