To determine the safety and efficacy of oral pentoxifylline (PTX) administration in improving semen parameters in infertile men with idiopathic OAT.

Why is it important?
Infertility affects approximately 15% of couples, and a male factor is implicated in up to half of these cases. In many cases, the underlying cause of male infertility is not fully understood and, therefore, the condition is considered idiopathic. With recent advances in reproductive technologies, infertility management has become increasingly successful. However, considerable numbers of couples still remain childless. Assisted reproductive techniques (ART) involve uncomfortable and invasive procedures, coupled with uncertainty regarding the outcome of treatment.
Perspectives
We have concluded that oral administration of pentoxifylline is an effective treatment for improving the semen parameters of infertile men with idiopathic OAT. Larger studies with different treatment regimens and a longer follow-up period are necessary to confirm the beneficial effect of oral pentoxifylline on semen parameters and pregnancy rate.
