The present study is the most comprehensive systematic review and meta‐analysis of observational studies evaluating the outcomes of darvadstrocel (a suspension of ASCs) delivered as a one-time local injection for complex perianal fistulas in patients with CD.

Why is it important?

the results of this meta‐analysis of observational studies confirm the effectiveness (with very small heterogeneity between studies) of darvadstrocel as a single injection delivered by a minimally invasive surgical technique in achieving healing of complex perianal fistulas refractory to standard treatment in patients with CD. The acceptable safety data presented here support the positive long‐term benefit‐risk profile of darvadstrocel.

Perspectives

The ideal treatment goal for complex perianal fistulas associated with CD is the closure of all fistulas and prevention of recurrence. However, these goals are often unmet despite intensive medical treatment (including infliximab) and conservative surgery. Therefore, management of treatment‐refractory complex perianal fistulas associated with CD remains a challenging problem. These real‐world effectiveness and safety results with darvadstrocel provide a broad perspective that can be used to guide treatment decisions in the management of patients with perianal fistulizing CD who have not responded to conventional medical and surgical therapies.
